Terms & Conditions for Payment & Cancellations
The following Terms and Conditions come into effect once a date for assessment has been confirmed.
As the person who has authorised Ruth Shattock (nee Clevely) to arrange a diagnostic assessment you will be deemed responsible for accepting and complying with Ruth Shattock’s Terms and Conditions once the assessment has been arranged.
Agreement on Terms and Conditions
1. Your work, college or university have agreed to fund the payment fees for your assessment of £395; you must advise immediately if this is not the case.
2. Ruth Shattock will conduct the assessment and confirms she is a suitably qualified Specialist Assessor, holding a current Assessment Practising Certificate (APC) producing diagnostic reports suitable for applying for Disabled Students Allowance (DSA).
3. The assessment will include a full battery of relevant tests and a full diagnostic report. This can take up to four hours and at least one break will be offered during assessment. The assessment is nothing to worry about and you can’t revise for it. The report will be produced within 10 working days of the assessment and will be sent electronically and will be password protected.
4. The client has the right to cancel a booked assessment up to 24 hours before the assessment date. In this case a cancellation fee of £150 will be incurred.
5. If the client does not attend the assessment booking then the work, college or university will be invoiced for the full amount of £395.
6. If Ruth Shattock has to cancel an assessment appointment you will be offered an alternative date if possible. If this is not possible you will receive a full refund.
7. The assessment report will be sent electronically and password protected to the person who has funded it. You will be notified when this has happened in order for you to arrange to receive the assessment report.
8. The invoice will be sent with the assessment report and your work, college or university have 30 days in which to pay the invoice.
9. Your data will be kept electronically, please see our Privacy Policy. Only the data which is needed to complete the assessment will kept and the data needed to contact you about the assessment. The data needed to complete the assessment will be kept electronically for 12 months from the date of assessment and then securely destroyed. In addition, an electronic copy of your report will be kept for a term of 6 years in secure files. Your electronic data is kept securely and is password protected. Your data will not be use for any other purpose than to produce the diagnostic report and to liaise with you regarding the assessment. It will not be passed onto third parties without your express instructions.
